I use a pair of Sony MDR-NC-11 buds (see this site for details). They wedge into your ear canal and the noise cancelling circuitry rescues about 10 dB of background. My only complaint is that the wires are a bit fiddly and that I have to remember to carry a couple of spare AAA's when I'm travelling. They work wonders in an ambient noise environment such as a bus, airplane, train, kids room.....
