When did AppleWorks start becoming a "Built in" app in new Macs?
Is it pre-installed or do you get a disk?

It's pre-installed, but chances are it's on disc as well. AppleWorks has shipped on several different Macs for a long time now.

It's only on the consumer line. It is usually on the restore application not the basic OS X install. For older macs there used to be a separate Applications disk that had it.

It began about 1993 when the Scully-led Apple corporation introduced the Performa as a "consumer" line (additional SW bundle) and the Centris/Quadra as the "Professional" line (no additional SW).

The iMac was carried over by the Jobs-led Apple Computer as a kind of "Performa" line. Desktop Tower Macs are considered "Pro" computers today.
