Have you updated to 10.5.6 yet? Since doing that the sound goes to sleep in 10 seconds now for me.

Yes, I am running 10.5.6. I just quit KeepSoundAwake and discovered that the popping through my external speakers is less frequent than before (however it is still happening). Guess I'll just keep running KeepSoundAwake for now.

The volume keys simply result in the "you can't do that" symbol overlaid on my screen (speaker with the stop sign w/ diagonal slash). System Preferences Sound pane also gives me no options.

Yep. I've had to do this before too...I actually used a pin and jiggled it around in there to trip the switch. This also used to happen on the 2G iPhones as well (although it didn't light up when this happened), the connection switch would get jammed by dirt or something.
