If I want to upgrade to Adobe Photoshop CS, do I have to have Photoshop 7 first? or can I go from Photoshop 5 to Photoshop CS?

According to this page on Adobe's site, a registered version 7 or earlier is fine. Scroll down to "Upgrade".

Adobe allows upgrades from any previous version.

You also have the option to upgrade from PhotoShop alone to the entire Creative Suite (PS, Illustrator, InDesign, Version Cue US$550/ or plus Golive, Acrobat Pro; US$750).

If you do upgrade to the suite you won't be able to upgrade to PhotoShop alone after that (when a new version ships in the future).
